What are the benefits of being in the Virtual Hospital?
- We can personalise your care to your needs.
- Use of remote monitoring technology can prevent readmissions into a hospital bed.
- Being in the comfort of your own home can improve your care experience.
- Being able to stay at home, rather than be in hospital can take the pressure off you and your support network.
- We communicate with your own GP to ensure your healthcare system supports you.
Other benefits include the reduced risk of hospital-based complications affecting you, and a more seamless transition into becoming independent once you are discharged.
Will I be safe?
The virtual hospital uses a sophisticated system that tracks your condition remotely, alerting the hospital team if it’s necessary to increase the level of care.
We will provide all the equipment you need and make sure you’re comfortable using it before you go home; the equipment is simple to use.
Depending on your condition, you may not need to do anything other than wear the equipment and the data will sent to us automatically and continuously. For other conditions, you may need to add information regularly to an app.
You will receive regular phone calls from our nurses and doctors.
What conditions can be cared for in the Virtual Hospital?
Currently we care for the following conditions in our Virtual Hospital:
- Heart Failure
- Acute Respiratory Infections
- Asthma, Bronchiectasis, COPD
- Frailty
- Colorectal Surgery
We plan to expand this list in the future and provide support to a wider range of people for a variety of different conditions.
Partnership
Our virtual hospital is run in partnership with the community team at Central London Community Healthcare NHS Trust, under the South and West Health and Care Partnership.
